How do I get a copy of my films and/or medical records?
When requesting a copy of the films, we will ask you to complete a Film RECORDS REQUEST FORM. Please fill out the form completely, including specific examinations needed, dates of service, your signature, and where the examinations will be going. In some instances we will require advanced notice of 24 hours to prepare your examinations for release. We will provide your primary care physician with his/her choice of film or CD.
